Your First Appointment

Welcome to Concord Eye Care, P.C. We are pleased that you have chosen our office to provide your eye care services. For your convenience, we have a full service Optical Department with an in-house optical lab to professionally make your personal eyeglasses, many times within one hour. Our staff is available to answer any questions that you may have.

At your first visit, please be aware of the following:

Plan at least one hour for the appointment.
A complete exam includes dilation, which may cause light sensitivity and blurred vision.
Children under 18 MUST be accompanied by a parent or legal guardian.

You will be asked to complete a Medical History Form and provide Insurance Coverage Information. To save time at your appointment, at least three (3) days prior please download and print the following forms, complete the information, and fax them back to (603)228-0248, ATTN: Patient Services Department. If you do not complete these forms in advance of your appointment, we ask that you arrive at least fifteen (15) minutes prior to your scheduled appointment time to complete the forms.

Please also bring the following information with you to your appointment:

Current eyeglasses and/or prescription
Current contact lens prescription (if applicable)
Current Insurance Card(s) �� Please review your insurance plan as it relates to your eye care benefits and whether or not you need a referral from your primary care provider (PCP). Co-insurance amounts, co-pays, deductibles, and any non-covered services, such as refraction fees and contact lens fees, must be paid at the time of service.
